NOTES:
1. Mileage is paid at 40 pence per mile for staff and 25 pence per mile for players
2. Players can claim room costs back
3. If a player is rooming with a carer they have taken then they can claim half the room back
4. When complete please return to the Treasurer for payment. Thank you.
